I hereby grant permission to Triple Ring Technologies, Inc., and the rights of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or video without payment or any other consideration in its publications, advertising, or other media activities (including but not limited to the internet, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other printed and digital publications). I understand that my image, likeness and recorded voice may be edited, altered, enhanced, cropped, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein my image, likeness or recorded voice appears. Additionally, I waive any right to royalties or other compensation arising out of, or related to, the use of such images, photographs, videos, audio or recorded voice. I understand and agree that any photograph, image, video, audio or recorded voice using my image, likeness or voice will become property of Triple Ring Technologies, Inc. and will not be returned.
This consent includes, but is not limited to permission to interview, film, photograph, image, tape, or otherwise make a video reproduction of me and/or record my voice; permission to use my name; permission to use quotes from interview(s) (or excerpts of such quotes), the film, photograph(s), image(s), tape(s), video(s) or reproduction(s) of me, and/or recording of my voice, in part or in whole, in its publications, in newspapers, magazines and other print media, on television, radio and electronic media (including the Internet and electronic mailing), in theatrical media and/or in mailings for fundraising, sales, marketing, educational, awareness and other lawful purpose.
I hereby hold harmless and release and forward discharge Triple Ring Technologies, Inc. from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I am at least 18 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release. This authorization extends to all languages, media, formats, and markets now known or hereafter devised and shall continue indefinitely.
This consent is given in perpetuity and does not require prior approval by me.
